Important! Deaths must be reported to ERSGA for all pension plans and GTLI benefits. For PSR 401(k), 457 Plan accounts, and Flexible Benefits, death notifications must also be reported to GaBreeze. A certified copy of the death certificate will be required by both ERSGA and GaBreeze for benefit processing.
